Students Under 19 will need parent/guardian Name and Email to complete this form. Once student signs, the form will be routed to the parent/guardian to complete.

Pre-Enrollment Health Requirement Form -Students Under 19
NCTA students are required to attach proof of two separate rubella measles/MMR immunizations or a positive rubella lab result. The immunization record typically can be obtained from your high school or health care provider. It is strongly recommended students complete this requirement before beginning classes. Students who do not complete the health requirement will have a hold placed on their account that will keep them from registering for classes the following semester. Pictures or scans of immunization records can be uploaded directly to this form.
These forms are not needed for enrollment, and should be completed before coming to campus that semester. They will need to be filled out each year you are on campus.

This form will be used by NCTA students to use the fitness center on campus, and to participate in Intramural Sports - Parking Permit - all faculty, staff and students with a car on campus must have a parking permit.
